Countywide Urgent and Emergency Care Teams Discharge to Assess Team (D2A)
This is an exciting opportunity to join the Countywide Urgent and Emergency Care Teams within Nottinghamshire. These teams cover Queens Medical Centre, City Hospital, Lings Bar, Kings Mill Hospital and Bassetlaw Hospital.
The Urgent and Emergency Care Teams follow the Discharge to Assess Model. Discharge to assess supports people to leave hospital, when it is safe and appropriate to do so. Our teams work as part of an integrated multidisciplinary transfer of care hub based within the hospitals to plan supported discharges with short term social care support as needed. Once discharged, the goal is for the person to continue their recovery, rehabilitation, and reablement, where we will then support with assessments to promote the individual’s independence and well-being.

All our work is underpinned in accordance with The Care Act 2014 and Mental Capacity Act 2005. Good Communication, time management and IT skills are essential for the role.
