**JOB PURPOSE**
Community Care Officers are required to complete Care Act assessments; face to face or telephone assessments, depending on the level of risk and the needs of the citizen being assessed, with consideration of mental capacity throughout. They work with supportive colleagues, citizens, carers, statutory and community partners to provide appropriate support that will enable independence, choice and control, promoting early intervention and local community led support services to enhance well-being and respond to local community needs.
**TEAM INFORMATION**
The Adult Duty Team comprises of Community Care Officers and Social Workers and we work with adults 18 years of age and over with a range of needs.
In the Adult Duty Team, we support adults and their carers by completing holistic, person centred assessments and have a strength based approach to our interventions. We support adults and carers to make informed choices and empower them to have control over their lives and the care they receive.We work closely and effectively with our partner agencies and our aim is to support adults to maintain their safety and independence in the community for as long as possible, with a focus on early intervention and prevention.
We pride ourselves on being a highly supportive team, with a range of experience and wealth of knowledge to share. We also welcome new ideas and experiences to contribute to the continuous development of our team. The Adult Duty Team is a great team to work in to expand your skills and knowledge due to the vast range of work we are responsible for.
We have a Hybrid model of working in Adult Duty and therefore you have the opportunity to work from home and an office base. We are based at the Mary Potter Health Centre, Hyson Green, Nottingham.
