At an exciting time of transformation at Norfolk and Suffolk NHS Foundation Trust, we are looking for experienced and compassionate clinical professionals to join our existing leadership team in Great Yarmouth and Waveney Care Group.

Building relationships across our teams as well as influencing the Mental Health Agenda across our developing Primary Care Networks, including third sector organisations forms an essential part of the role.

Providing excellent communication and network links with system partners at a time of transformation and investment for Community Services, the role supports Community Transformation in line with the national long term plan. Building and supporting seamless pathways in community care and strong links and good reputation building with primary, secondary and VCSE colleagues for our Front Door service, primary care practitioners and Learning Disability across Great Yarmouth and Waveney.

Supporting the strategic and operational planning as well as leading delivery across Waveney and Great Yarmouth Community Services, the successful post holders will influence the clinical and operational direction and development of our Front Door and PCN teams

The post holder will work closely with our partners to ensure responsive, high quality service to clients, their families and carers and meeting commissioner and stakeholder expectations.
