Doncaster, United Kingdom Flourish at Woodfield Park Full time

**ORGANISATION OVERVIEW**

Flourish Enterprises is a CIC based in Balby, Doncaster. We work out of a stunning listed building that provides inviting office space, conference and function hire, large scale community events and volunteering opportunities, historic walled garden & shop and cafés all surrounded by more than 21 acres of mature woods, wild meadows and open green spaces.

As a social enterprise we focus on providing care and support services throughout our local communities. We exist to promote social mobility, wellbeing, opportunities and work with our partners to deliver a positive and lasting social impact.

We provide work and vocational opportunities to people with mental health needs, learning disabilities, neurological conditions and to those that have experienced other forms of set-back and who need support to get back into education and employment.

Our range of activity includes Woodfield 24, our specialist end-of-life care service providing community-based palliative care support to patients and their families - ensuring that their remaining time can be spent together.

As a not-for-profit organisation we ensure that every penny of surplus generated from our services is reinvested back into community led activity. We are fully owned by the RDaSH NHS Trust and together we share the same site at Woodfield Park.

**ROLE SUMMARY**

Following an organisational review, Flourish is now looking to recruit an enthusiastic and ambitious individual to fill a critical position within our community services.

The postholder will enjoy a significant degree of autonomy and creativity to design and oversee a range of programmes and community focused activity that promotes accessibility, inclusivity, opportunities and togetherness, whilst offering direct operational support to the senior leadership team.

This role also involves direct supervision of two other people within the organisation - our Community Connector (10 hours per week) and LTA Tennis Activator (5 hours per week).

- You will be the first point of contact for referrals/enquiries about our community support programs, and you will ensure people are provided with accurate and up to date information about Flourish and about other activities/services in the area;
- You will carry out brief informal assessments to identify people who need higher levels of support, and help them to access the appropriate healthcare services;
- You will encourage and promote healthy lifestyle choices at every opportunity;
- Support the delivery of our contracted projects by keeping accurate records, completing case studies, and providing information on outcomes and social impact as required for reports;
