The City of Doncaster Council is a confident, ambitious organisation which puts improving the life of its residents at the centre of everything we do. You have a opportunity to be part of this in the role of Community Care Officer in the Integrated Discharge Team, Home First Directorate.
This is a multi-disciplinary team where Social Care staff work alongside Discharge Nurse Specialists, Therapists, Older Peoples’ Mental Health and Community Nursing services to assess and plan discharges from the acute setting following a range of discharge pathways. We also work in the Emergency department and 2 rehabilitation hospitals to reduce and delay the need for formal support and to prevent hospital admissions with support from community colleagues. The role will focus on discharge planning including assessments of capacity, consideration of continuing health care eligibility and primarily consider returning home if feasible and safe to do so.
The team works across all pathways with our focus being “Why not Home Why not today” completing post discharge conversations taking a strengths-based approach promoting independence and reducing and delaying needs where possible. Connecting the person to their community and care and support when longer term needs are identified, engaging with professionals in a timely way to get the outcomes that are right for the individual.
Applicants are welcomed from people with relevant experience of assessment under the Care Act 2014, Mental Capacity Act 2005, Hospital Discharge policy 2020, GDPR, and awareness of safeguarding. A Home first focus is used to support timely discharge of adult patients from the acute hospital. Skills and knowledge in recording, report writing, strength based approach to assessment, risk assessments and an aim of positive outcomes are required for this role. You will also work with carers to support, signpost and refer for assessment of their needs in accordance with the Care Act 2014.
This is a very fast paced environment working to short timeframes and requires an organised approach and ability to work with several patients and families concurrently.
