Social Care Advisors

Warrington, United Kingdom Warrington Borough Council Full time

An exciting opportunity has arisen to join the One Front Door team as a Social Care Advisor.

The One Front door service is the single point of contact for all of Warrington residents needing to access Warrington’s Intermediate Tier services. Working together, our Health and Social care services have created a “One system, One Population, One Purpose” to improve the outcomes for our Warrington Residents.

The team is the first point of contact for all new referrals coming into Adult Social Care, the reporting of Safeguarding Adult Concerns and the Local Authority’s duty for the provision of the AMHP service to coordinate assessments under the Mental Health Act (1983 as amended 2007).

The Social Care Advisors are responsible for ensuring all initial assessments for adults and consequent service provision meet the Council’s statutory responsibilities, including the responsibility to promote an individual’s well-being. The Social Care Advisors have a key role in appropriately managing the demand into Adult Social Care, promoting Information, Advice and Guidance, and ensuring all contacts and assessments fully utilise the **_Strengths Based Approach model to Adult Social Care and to engage in ‘good conversations’_**_ with Warrington people._

All Social Care Advisors work closely with other professional, voluntary and non-statutory agencies offering information, advice and guidance, including sign posting to alternative community based provision, for those referrals that are not eligible for adult social care as defined by Care Act 2014.

Applicants must have excellent communication, interpersonal and IT skills and be able to demonstrate a high level of commitment to customer care.

Operational hours of the service are office hours Monday to Friday from 08:30 to 17:00. This is a full time 37hrs post that is available for job share.

Enhanced DBS clearance will be required

DBS clearance is required.

The ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
