Role Summary

Are you looking to challenge yourself and provide support and co-ordination to a small, but busy team of dedicated clinicians who provide care for our elderly patients' resident in care and nursing homes?

An exciting opportunity has come up for a Care Co-ordinator to work as part of the Beacon Medical Group Care Home Team. This is a fixed-term appointment to provide maternity cover for our current Care Co-ordinator.

The successful candidate will be self-motivated and able to work with minimum supervision, planning and co-ordinating the day-to-day work of our Care Home Team as well as supporting Multi-Disciplinary Team Meetings, rota management, liaison with outside agencies, including care homes, District Nurse Teams, running and compiling reports and other administrative tasks.

Main Duties of the Role

ACCOUNTABLE TO: Operations Lead

Key Responsibilities:

Have to work closely with Practice & Care Home teams in a professional manner to nurture a spirit of collaborative team working.  Support the Multi-Disciplinary Team (MDT) meetings and the ongoing patient case management.

Work effectively alongside other teams and develop positive working relationships with colleagues and external stakeholders.  Promote a positive working environment. The Care Coordinator role is seen as a critical and evolving post to support the delivery of Enhanced Health in Care Homes within the PCN.

The duties and responsibilities to be undertaken by the Care Home Co-Ordinator may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the Operations Manager, dependant on the current and evolving workload and staffing levels.

To be responsible for your own continuing self-development, undertaking training as appropriate.

To undertake other duties appropriate to the grading of the post as required.

Must be able to work flexible hours.

Patient/Customer Care:

Registration of new patients in the care homes and ensuring correct packs and processes are completed.

To assist the care homes with set up of patient proxy access for online services.  Process telephone requests for appointments, visits, medication support and telephone consultations.

Support national screening and immunisation programmes.

Co-ordinate the delivery of weekly ward rounds for the clinical team and be point of contact for the care homes for these.

Ensure that all patients' Care Plans, Treatment Escalation Plans, DNARs and associated reviews and patient-related correspondence are available at the MDT meetings, liaising with all stakeholders as appropriate, accessing & auditing IT systems to ensure relevant information is available.

Co-ordination:

Under guidance from the line manager, take initiative in the organisation and administration of MDT meetings, tasks relating to the co-ordination of the Care Home team services, rota management, meeting planning, minute taking, review of audits and training support to minimise the demands upon the multidisciplinary team.

Manage agenda items, ensuring all new referrals and cases for discussion are identified and information circulated to team members in advance of the meetings and keep clear minute records of MDT and any other care home related meetings and maintain action logs related to this.

Deal with complex and sensitive issues to ensure administrative and clerical duties within the Practice are carried out within the Practice's guidelines and policies. Support patients to utilise decision aids in preparation for a shared decision-making conversation ensuring efficient use of time and available technology.

Navigating patients through their care pathways across a multitude of topics (including but not limited to: Cancer, Diabetes, COPD, Dementia).

Create, amend, and use searches within the clinical system to identify cohorts of patients.

Ensure that required clinic templates are correctly entered onto the system and are visible 6 weeks in advance.

Liaise with the Care home GP, PCN Clinical Pharmacist and Operations Manager regarding any changes to clinics due to annual leave or sickness.

Maintain Care home lists and ensure the registration lists are managed appropriately.  Act as a point of contact for community service providers and co-ordinate multidisciplinary care.  Maintain protocols for care homes, for information sharing, shared care planning and governance of records.

Explore and utilise available digital technology and support the homes with the use of this.

Ensuring personalised care and support plans are kept up to date.

Ordering: this may be stationery and office supplies, consumables or medical/clinical supplies ordered under the supervision of one of the Nursing and dispensary co-ordinator.

To maintain a thorough knowledge of all Practice procedures and work in accordance of written protocols.

Support Quality Outcomes Framework and other LES/DES specifications and work with management team to coordinate the delivery of these.

Co-ordinate seasonal requirements and vaccination programs such as Flu.

Other tasks:

Undertake a variety of SystmOne tasks to assist in the smooth running of the service, contacting patients/care homes as requested by managers and clinical staff when required. Responding to system wide alerts.

Contributing to the development and improvement of services to patients.

To assist your colleagues in learning new procedures and offering support across the team when required.

Assist in devising and getting agreement on multi-agency guidance to support the MDT function.

Person Specification

Qualifications:

Essential: Evidence of a good general standard of education. Good IT skills.
