Band 6 Registered Nurse – Hospital at HomeWe are looking to recruit a Band 6 Registered Nurse for the Hospital at Home service, providing urgent community response and virtual ward care to West Cheshire residents in their own home. You will be joining a multidisciplinary team including GP, Advanced Clinical Practitioner, nurses, therapists, pharmacists and clerical officers.We are looking for someone who is enthusiastic, self‑motivated and provides confident support and leadership and coaching to clients and clinicians. You must have excellent communication skills and the ability to work in partnership with clients and their families/carers and, when necessary, act to safeguard individuals who are at risk from harm and abuse.This is a permanent post working 30 hours per week across a 7‑day service, between 8am and 8pm.Key ResponsibilitiesProvide a highly visible clinical profile with leadership and clinical input for a designated clinical caseload.Work collaboratively with the wider health, social care and third sector partner agencies to meet the needs of patients.Ensure patients receive the right care, by the right person at the right time and the right place.Place the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, with minimal avoidable delays, and an emphasis on people achieving independence whilst improving quality of care.Provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives.Be aware of local public health community profiles and how this influences the required care for the population.Focus on the delivery of person‑centred care.Provide effective, evidence‑based care to a defined population through caseload management and monitoring of care standards.Provide clinical leadership, coaching and development of the clinical team, where appropriate.About the PartnershipCheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all‑age disability care, as well as the provision of three GP surgeries in Cheshire.
