Clinical Lead Community Nursing The closing date is 09 December 2025 We are looking to recruit a Band 6 Registered Nurse for the Hospital at Home service, providing urgent community response and virtual ward care to West Cheshire residents in their own home. You will be joining a multidisciplinary team including but not limited to GP's, Advanced Clinical Practitioner, nurses, therapists, pharmacists and clerical officers. We are looking for someone who is enthusiastic, self‑motivated and who provides confident support and leadership and coaching to clients and clinicians. You must have excellent communication skills and an ability to work in partnership with clients and their families/carers and when necessary, act to safeguard individuals who are at risk from harm and abuse. This is a permanent post working 30 hours per week across a 7‑day service, between 8am and 8pm. Shortlisting for this vacancy is due to be completed on 11/12/2025 and the proposed interview date is 18/12/2025. Main duties of the job Provide a highly visible clinical profile with leadership and clinical input for a designated clinical caseload. Working collaboratively with the wider health, social care and the third sector partner agencies to meet the needs of patients. Ensure patients receive the right care, by the right person at the right time and the right place. Placing the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, with minimal avoidable delays, and an emphasis on people achieving independence whilst improving quality of care. To provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives. To be aware of local public health community profiles and how this influences the required care for the population. To focus on the delivery of person centred care. To provide effective, evidence‑based care to a defined population through case load management and monitoring of care standards. Provide clinical leadership, coaching and development of the clinical team, where appropriate.
