The NHS is increasingly introducing virtual wards to support people at home, including care homes. In Cheshire West, the ‘Hospital at Home’ service will expand its offer with the implementation of UCR and virtual wards allowing patients to get the care they need at home safely and conveniently.

The Hospital at Home workforce supports hospital level care at home; General Practitioners, ACP's, registered therapists / nurses and support workers offer a 7-day service from 8am-8pm, allowing for wrap around care & an excellent patient experience.

We are looking for a hard-working, enthusiastic, highly motivated & experienced Advanced Clinical Practitioner to join our Hospital at Home team to support this innovative new way of working.

The primary focus of the role is to provide patient-centred clinical care, encompassing assessment skills & diagnosis of patients and providing ongoing acute management under the medical oversight of the GPs within the Hospital at Home team. The posts are intended to provide improved outcomes for our patients by providing safe, kind and effective care.

The post is part time 22.5 hours per week.

We are looking for a hard-working, enthusiastic, highly motivated and experienced Advanced Clinical Practitioner from nursing/AHP to join our Hospital at Home team to support this innovative new way of working.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. Please note that DBS costs will be waived for applicants who are being recruited for COVID-19 reasons.
