Key Responsibilities Identify suitable patients in the acute setting using current pathways, supporting early supported discharge through the Hospital at Home service Conduct community assessments as needed and collaborate with multidisciplinary team (MDT) members to develop personalised care plans Monitor patients remotely in accordance with standard operating procedures, ensuring safe and effective virtual care Apply evidence‑based clinical skills and utilise digital technology to deliver high‑quality care in patients' own homes Manage your patient caseload effectively, ensuring smooth handovers of care and maintaining clear, timely communication Develop and sustain seamless referral pathways to other agencies and specialist teams, fostering strong MDT collaboration Work within your scope of practice and clinical competencies, adhering to professional policies, national standards, and Trust procedures Contribute to ongoing service development and quality improvement initiatives to enhance patient care The Northern Care Alliance NHS Foundation Trust (NCA) provides hospital and integrated health and social care services to over one million people living across Greater Manchester.
