Hospital & Care at Home Autonomous Practitioner

The post holder will work within the multidisciplinary team to deliver high-quality care, providing clinical and managerial leadership in the absence of the clinical lead. You will organise the care environment to meet patient needs, improve their experience, and support staff in achieving divisional and Trust objectives.
**Key Responsibilities**:
Provide expert clinical assessments and deliver high-quality care within established protocols
Utilise evidence-based clinical skills and digital technology to care for patients in their own homes
Manage patient caseloads, ensuring effective handover of care and timely communication
Develop and maintain seamless referral pathways to other agencies and specialist teams via the multidisciplinary team (MDT)
Work within your scope of practice and clinical competencies, ensuring compliance with professional policies, national and local guidelines, and Trust procedures
Play a vital role in delivering and developing formal and informal education for MDT members
Offer specialist advice and support through advanced clinical reasoning and evidence-based practices
Contribute to service development and improvement initiatives.
Work autonomously, assessing patient needs and making informed clinical decision
