We call this “Our Nottinghamshire Approach”. We are looking for passionate and experienced individuals to join our growing Family Hub Teams in Nottinghamshire, supporting families with children aged 0–18, including those with SEND. About the Role As a Family Support Practitioner, you will ensure that families receive appropriate support services from conception until the child reaches age 18 or 25 with SEND. You will deliver and review family support services, one‑to‑one support, evidence‑based parenting programmes and interventions within a defined locality to keep children safe and help families thrive. Key Responsibilities Provide information, advice and guidance for expectant parents and families with dependent children (up to age 19). Carry out structured whole‑family assessments and create support plans to address family needs. Work with internal and external services within the Family Hub Network to meet wider family needs. Undertake targeted one‑to‑one work with parents/carers to deliver evidence‑based interventions and programmes. Act as a lead professional for families requiring coordinated support packages involving several services. Support parents/carers in developing parenting skills and implementing routines appropriate for their child’s developmental stage. Maintain record‑keeping using case management and business intelligence systems and contribute to reports. Measure impact through regular reviews of assessments, support plans and tracking. Co‑facilitate interventions with key partners (Children’s Social Care, health partners, schools, voluntary sector). Maximise involvement of parents/carers in evaluation using a range of tools.
