We have multiple exciting opportunities to join our Family Hub Teams in Nottinghamshire As we expand our service offer from supporting children aged 0–5 to now include children and young people aged 0–18, we are looking for passionate and experienced individuals to join our growing teams. We are particularly keen to recruit candidates who have experience working with families who have children aged over 5 years and teenagers. As a Family Support Practitioner, you will ensure that families with identified needs receive appropriate family support services within the home and community to ensure they get the right help at the right time from conception until their child reaches the age of 18 or 25 with SEND. You will deliver and review a range of family support services, one to one support, evidence-based parenting programmes and interventions within a defined locality to help keep children safe and help families thrive. Posts Available Ashfield District - Hucknall Family Hub – 1 × 37 Hour post (Full Time), 1 × 25‑hour post Kirkby Family Hub 2 × 37 hour posts (Full time) Broxtowe District – Eastwood Family Hub 1 × 30 hour post Key Responsibilities Provide information, advice and guidance for expectant parents, and families with dependent children (up to the age of 19) to help them navigate and access services that meet their needs as early as possible. Carry out structured whole family assessments and create support plans to better understand and address the needs of families and help them to thrive. Work alongside internal and external services within the Family Hub Network to ensure the wider needs of families are addressed by the most appropriate service (e.g., health). Undertake targeted one-to-one work with parents/carers to meet identified needs and deliver evidence-based interventions and programmes, within dedicated buildings, communities, and family homes. Act as a lead professional for families who require additional support and a coordinated support package which is likely to include several services and interventions. Support parents/carers in developing their parenting skills and in implementing routines appropriate for their child’s stage of development through the delivery of a range of interventions and family support. Maintain record keeping using case management and business intelligence systems and contribute to reports as required. Measure the impact of support provided through regular reviews of assessments, support plans and effective tracking using case management systems. Co-facilitate interventions with key partners within the Family Hub Network including Children’s Social Care, health partners, schools, and the voluntary sector. Maximise the involvement and participation of parents/carers in the evaluation of interventions and programmes using a range of tools.
