All Age Continuing Care delivers the patient specific Learning Disabilities and Autism Programme Integrated Care Board responsibilities.

Main duties of the job This is a challenging post which will require dynamic leadership and excellent communication, risk mitigation, data analytic and financial management skills along with a proven capacity to work across the multidisciplinary spectrum to engage key stakeholders to deliver the NHS All Age Continuing Care agenda.
This is a key tactical post straddling the strategic/operational interface within NHS All Age Continuing Care services.

Person Specification Knowledge, Training and Experience Essential Registered Health or Social Care Professional with current registration with professional body OR significant relevant leadership/management experience/qualifications.
Educated to Masters level or equivalent level of experience of working at a senior level in NHS All Age Continuing Care.
Excellent understanding of the structure of both the NHS and Social Care, including commissioning and planning processes.
Advanced knowledge and understanding of All Age Continuing Care policies and other legal frameworks as required (Children and Complex Care, s117 Aftercare, Mental Health Act etc.)
Court of Protection/legal case management and practice within the NHS, including the strategic context within Health and Social Care.
