Role Summary We have two exciting short term opportunities for dynamic and forward thinking clinical leaders with experience in community.
You will play a pivotal role in working collaboratively with acute and community colleagues in order to effectively deliver outcomes for patients who are ready to leave hospital and need support to do so in a timely way.
The ideal candidates will demonstrate and evidence passion for the role, agility, team work and a positive risk appetite with credible clinical judgement.
Acting as role model to encourage and challenge decision making at all levels, you will be expected to provide professional advice and guidance to others and always demonstrate a 'can do' attitude.

More detail about the role You will be based at Wonford Hospital and be working clinically on the wards, liaising directly with clinicians in the acute, the community or care homes, depending on the focus of your particular pathway.
You will need to work at a fast pace and make rapid decisions about the most appropriate care for individuals, ensuring that risk appetite is proportionate to pressures across the patient pathway.
