Thisinvolves being responsible for assessment, developing, implementing andevaluating care plans using a person-centred approach.
Ourideal candidate would be a committed team player as well as being able to workindependently demonstrating initiative and passionate in promoting good qualitycare to maximise independence.
Within this role you will gain training,support, and guidance to ensure you are working collaboratively with externalagencies to give the best possible care for our service users.
Main duties of the job This rolerequires someone who can manage in a fast paced environment completingassessments to support diagnosis.
The day to day work for this role will be ledby the needs of the service users and the service which will require effectivetime management and prioritisation skills as well as effective communicationand autonomous working.
Allof our staff are an integral part of the multidisciplinary team within mental health services.
The successfulcandidate will need to understand the importance of engaging with externalagencies within our area in order for our service users to achieve high-qualitycare that we in Navigo always strive to deliver.
This should involve usingsupervision and reflecting on practice to develop new and improved ways ofworking.

Communication &relationship skills Use highly developedcommunication skills to conduct assessments of service-users and communicatecomplex information related to their mental health needs, to service-users,carers, relatives and other professionals.Effectively communicatecomplex and/or sensitive information where there are barriers to understanding.This includes communication concerning adult protection, abuse, cognitiveimpairment and other risks surrounding a clients mental health.
Barriers tounderstanding include working with service-users who could be hostile or highlyemotional, who have complex needs and may be vulnerable.
Maintain and develop effectivecommunications and working relationships within the team and with externalagencies.Promote and implement creativeways of working with service-users Instigate, develop andmaintain a relationship with the client and/or carer to facilitate the accurateassessment and delivery of care.Manage highly confidentialinformation in respect of members of the public in accordance with local andnational policy.Act as care co-ordinator andnamed worker responsible for specialist assessment.2.
Knowledge, trainingand experienceThe post-holder will holdcurrent registration on part 3 or 13 of the NMC register or hold a RegisteredMental Nursing (RMN) certificate and Social Work qualification CQSW, CSS orDipSW or be a Registered Occupational Therapist.Have specialist knowledge andtraining of working in a Memory Clinic, gained through formal training and postregistration experience and/or training, relating to older people and mentalhealth and apply this knowledge when delivering care to service-users.Provide advice and support toother agencies around issues relating to Mental Health.

Analytical &judgement skillsMake decisions about the rangeof options for treatment and care plans based on judgement of clients need.
Conduct comprehensiveassessments of both service users and carers in line with team operationalpolicies.

Planning &organisational skillsPlan and co-ordinate care planreview meetings with colleagues in the team and with staff in other teams andagencies, ensuring that review meetings take place at the appropriate time.Co-ordinate transfer orreferral of service-users to other teams or services where required.
Participate in AMHP Rota, BIArota if appropriate.Prioritise, plan andco-ordinate own workload and activities in line with the changing needs of theteam and service.
This includes planning and monitoring client medicationneeds, making appointments for service-users, conducting assessments, managingemergencies and enquiries, responding to crisis work within individualcaseload, ensuring patient records are up to date and supporting colleagues ona daily basis.
Have the ability to workindependently and to be able to resolve complex professional problems.Crisis response to otherstaffs service-users in the event of key workers absence, and delegation frommanager.Participate in caseloadmanagement and ensure relevant information is available for this to take place.5.

Responsibilities forpatient/client careDevelop programmes of care forservice-users and act as care co-ordinator for service-users on CPA, non-CPA,ensuring links with Adult Social Care.Assess and reviewservice-users clinical risk management plan using the DICES risk assessment.Assess care needs ofservice-users and carers to develop and implement appropriate care plans.
Provide specialist clinicaladvice to service-users, their carers, relatives and other professionals onneeds relating their difficulties and mental health.
This includes a range ofhealth education and advice.Initiate individual carersupport where required.
Set objectives, monitor and audit effectiveness.Participate in clinicalgovernance activities.7.
