Job summary ABOUT THE ROLEWe have an exciting opportunity for a Care Coordinator to join our dynamic team to provide maternity cover for a year.
We are looking for someone who is well organised and enthusiastic about providing high quality care to patients.
The role will mainly involve supporting our PCN Care Homes Team and the Care Homes to organise and support the weekly care home rounds.
You will also work closely with the practices and the PCN Business Manager to monitor the achievement of key performance indicators and requirements of the PCN contract which will involve direct patient contact.
The successful candidate must be confident in skills and knowledge to manage their own workload independently, finding solutions to issues when appropriate.You will be working remotely but may be asked to travel to Newark for meetings as required.

You will be part of our PCN Care Homes Team and supported by our PCN Business Manager.Interview date 23/01/2025.
Main duties of the job KEY RESPONSIBILITIESAdministration support to the PCN Care Homes Team including daily contact with Care Homes, adding rotas and appointments to the practice appointment system and reviewing outcomes from daily home rounds and actioning as appropriate.
Identify and refer new Care Home residents and hospital discharges.
Running monthly reports on staff activity and key performance indicators for the PCN.
Support the Care Homes Team, PCN staff and Practice managers with administrative work including contacting patients and booking appointments through an appointment system, creating and amending rotas etc.KEY REQUIREMENTSGood standard of education GCSE or equivalent in English and Mathematics Excellent working knowledge of Microsoft Office software (previous experience of using electronic patient record TPP SystmOne would be desirable) Understanding and able to deal with confidential and sensitive issues when liaising with team members, patients and other professionals About us PICS is the employer for this role.

To process referrals for and support PCN Social Prescribing Link Worker(s) To attend and minute the monthly PCN meeting, as well as providing a monthly update on the IIF figures To run monthly and quarterly reports on PCN Staff activity and present this in a Powerpoint presentation To run monthly reports on Investment and Impact Fund (IIF) targets for the PCN and circulate this to practices Provide coordination of and participate in relevant internal and external working groups and provide project advice, expertise and support where requested To participate in the review and appraisal process To carry out any other reasonable duties as requested by a manager to ensure quality of service To participate in any relevant training/courses/conferences Complete mandatory trainingThe post holder will be able to:Use clinical systems for record keeping, audit and evaluation Develop and implement data collection systems that will provide accurate and timely data Maintain confidentiality Person Specification Qualifications Essential Please provide details of your CPD, using examples where necessary.
