This is a full time position and will require working alternative weekends and a share of out of hours phone duty as part of a wider team.

Responsibilities Assisting with the creation of rotas and working patterns for all staff to ensure all service is covered with the support of a Care Manager.
Coordinating service delivery daily which includes managing inquiries from staff, clients and other health professionals we work alongside.
Undertaking formal checks on staff in the field.
Undertaking initial care assessments and creating and updating client care plans.
Contribute towards the delivery of a high standard of care through effective communication and teamwork.
Provide cover delivering care in contingency.
Requirements Requirements Must have a full UK driving license and car.
Must have a smartphone.
