Reminiscence Co-ordinator

**The Role**
- Leading and supervise the dementia care team in delivering excellent care
- Manage and supervise the care assessment process
- Case managing residents and liaising with families, healthcare professionals and other stakeholders to formalise bespoke individual care plans and risk assessment evaluations
- Work alongside activities manager to design, schedule and facilitate an appropriate programme for residents who have dementia
- Ensuring residents remain as active and independent as possible
- Provide a fun, inclusive, and professional approach at all times

**About You**
- NVQ Level 4 or 5 in health and social care and/or relevant experience is essential
- NVQ/QCF Level 2 or 3 in Dementia Care or equivalent is essential
- A detailed knowledge of care plans and advanced care plans in respect of end of life care
- Above all, have a natural ability to help make the lives of others more fulfilling.
- Proven experience in long term elderly care with a sound understanding of dementia care
