To facilitate this, we are creating a new role of Mental Health Care Coordinator.
Could this be you?This new role aims to help people of all ages manage their mental health needs better, whether it is parents needing support with parenting skills, families aiming to support their children and young people with mental distress or behavioural challenges, young adults transitioning from childrens to adult services, people of all ages needing help to navigate the mental health care services, or older people looking for support with issues of loneliness and isolation, bereavement or dementia.
This role aims to support them all.ABOUT YOUYou are passionate about advocating for the improvement of mental health and wellbeing for our population.
Motivated to empower individuals to improve and maintain their mental health, and supporting vulnerable people to navigate services to access the care they need.
You will be enthusiastic, flexible, innovative and proactive, with excellent communication skills.
Keen to work as part of a team, you will be self-motivated and capable of working autonomously to deliver specified outcomes.
You will be open to learning new skills and ideas, and to developing this new care coordination role to its maximum potential.
You will ideally have experience of working in health or care settings.
Main duties of the job We are looking for someone to join our existing team working hours per week at our PCN base within Ludlow Youth Centre.You would develop an extensive knowledge of and links with all the available mental health and wellbeing services in our community, both secondary care, community and voluntary sector, and work closely with colleagues in these organisations, as well as with our GP practices and PCN teams.You will work with individuals and families to assess their needs and signpost to the most appropriate services to help them, and act as a central point of contact to facilitate ongoing care.You will help to coordinate multidisciplinary team working, and facilitate some PCN services, such as drop-in clinics delivered by our Healthy Living Advisors.You will be encouraged to develop and deliver new ideas and services to address unmet needs which you identify through the course of your work.
About us South West Shropshire PCN is made up of 6 practices covering over 36000 patients over a large rural catchment area.
The population is generally older than average, with a higher proportion of frail patients.
Rural deprivation is a significant issue.Our Mission:Working together to build a bright future for rural general practice at the heart of healthy communities.Our Vision: We will collaborate and innovate to provide modern sustainable general practice to our communities.We will work with and coordinate health and social care services around practices and communities.We will improve children's and adults mental health.You will be employed on behalf of SWS PCN by Our Health Partnership.Our Health Partnership was set up by local GPs who arepassionate about providing high quality primary care and using their time andskills effectively to benefit patients.We are currently a GP partnership of 30 practices with 39sites, serving around 280,000 patients in Birmingham, Wolverhampton, andShropshire.If you have any questions regarding this vacancy, please contact:Deborah Shepherd, PCN Clinical Director: DorDebbie Davies, PCN manager: D Job description Job responsibilities The duties and responsibilities to be undertaken by the Mental Health Care Coordinator may include any or all of the items in the following list.Duties may be varied from time to time under the direction of the PCN management team, dependent on current and evolving PCN workstreams/priorities:Coordinating, integrating and delivering support to patients of all ages with identified mental health needs.Work with patients, their families and carers, to improve their understanding of mental health and wellbeing, and support them to access the appropriate services.Help people to manage their mental health needs through answering queries, supporting the making and managing of appointments, and ensuring that people have good quality written or verbal information to help them make informed choices about their care.Work with people, their families, carers and healthcare team members to encourage effective help-seeking behaviours.Help people transition seamlessly between services and support them to navigate through the health and care system.Work collaboratively with GPs and other primary care professionals within the PCN, as well as liaising with a broad range of medical, social and third sector services.Work closely with social prescribing link workers, health and wellbeing coaches, mental health support workers and other primary care professionals, helping to ensure patients receive a joined-up service and the most appropriate support.Regularly liaise with the range of multidisciplinary professionals and colleagues involved in the persons care, facilitating a coordinated approach.Actively participate in multidisciplinary team meetings in the PCN as and when appropriate.Support practices and the PCN in developing effective communication channels between GPs, people and their families and carers, and other agencies.Support delivery of PCN services to people with mental health needs in the community.Support practices with the delivery of mental health and learning disability annual health checks.Identify opportunities and gaps in the service and provide feedback to continually improve the service and contribute to business planning.Work in accordance with the practices and PCNs policies and procedures.Contribute to the wider aims and objectives of the PCN to improve and support primary care.Support in the delivery of the PCN Network DES, enhanced services and other service requirements on behalf of the PCN.
