The Direct Access to Wellbeing service (DAWS Team) is a brand-new project team that is part of the wider BCHC Charity.
The post will involve supporting individuals while in the care of BCHC, following discharge to home, at community events and following referrals from our partners, to offer advice and guidance, along with practical measures to support people to live better for longer and to be well and warm at home.

Main duties of the job The Community Wellbeing Officer is a key role within the BCHC Charity Project 'Keeping Warm and Well'.
As part of the Direct Access to Wellbeing (DAWs) Team the post holder will deliver early intervention signposting and support to patients, their families and the wider community in which BCHC operates.
The post involves having an awareness of BCHC health services and the wider NHS Support available allowing you to sign post individuals as required, offering Health and wellbeing guidance, early intervention health messages such as healthy eating, oral health and falls prevention.
The CWO role will also include offering energy saving tricks and tips, signposting individuals who are living in fuel poverty as well as offering a simple benefit check and referral for further support including the Priority Service Register (PSR)as needed.
Demonstrate a high level of communication and interpersonal skills in dealing with a wide range of NHS staff and external agencies and organisations.
The post holder will engage with individuals face to face and via telephone offer signposting, support, equipment, funding and additional items as required.
The post holder will link in with the wider BCHC services, NHS services and voluntary sector to ensure individuals are aware of available resources with the BCHC profile.

Understanding the needs of the beneficiaries and collecting necessary information to offer the appropriate support including signposting and referrals to external agencies.
Manage feedback and complaints from patients or individuals and seek to resolve issues in a proactive and patient or individually orientated manner.
Communicate effectively, appropriately and compassionately with patients, carers and families via phone and face to face in relation to all project activities.
Work alongside services to ensure the direct access to wellbeing model is understood and embedded to enable patients and their carers to receive the support available to help individuals live better for longer and supporting people to be warm and well at home.
Produce information in accessible forms taking account of the diverse needs of our patients and individuals, seeking support from wider BCHC services as required.
Engage with individuals to give specific advice or complete tools related to the project outcomes including but not limited to health services, energy efficiency and initial benefits screening.
To support stock levels ensuring items are regularly ordered and to raise requisitions via external support partners as specified by the senior project staff.
To support in the scoping and delivery of events, ensuring the project has a visible presence at events to maximise impact.
Record, analyse and monitor evaluations following attendance at events or after a contact ensuring the projects predetermined goals have been achieved.
Maintain confidentiality in all aspects of duty including patient and carer data.
Support junior staff with any training needs and provide direction and supervision as required.
To delegate tasks appropriately to junior staff as needed in terms of team generic inbox, leaflet packs, venue bookings, calendar planning etc.
This may require allocating and re-allocating tasks to meet organisational and team delivery and requirements.
To provide senior staff with regular updates flagging where there are issues regarding Individual and team capacity/ venue issues/ issues with ability to meet demand as pre-determined by the project.
To co-ordinate, monitor and problem solve scheduling of the wider project activities where staff are off sick or there is a clash in appointments, by liaising with senior staff to reduce cancellations of activity.
To follow agreed branding and formatting of materials and standards within the BCHC Charity and project branding agreements.
To support with monitoring of the team generic email in box, responding appropriately and in a timely manner.
To provide support to the wider BCHC Charity as required.
Engaging with high level administration duties such as answering phone calls and emails, organising of paperwork and responding to queries.
To promote the wider charity objectives and activities to increase the reach of the charity and its impact Attend and participate in team meetings and other ad hoc meetings when necessary.
Person Specification Qualifications Essential Educated to GCSE level or equivalent NVQ Level 3 qualification or equivalent Desirable Evidence of continued learning and development Experience Essential Relevant experience of speaking to people regarding sensitive subjects.
Relevant experience of working within the voluntary or health sector Experience in supporting the delivery of projects within a timeframe Experience supporting individuals with health needs or those most vulnerable in society Desirable Experience with completing referrals and signposting Experience working city wide across a number of bases Skills Essential Able to prioritise, multi-task and work well under pressure Excellent written and verbal communication skills Experience of working effectively without close supervision including dealing with problems as they arise.
Intermediate IT skills in MS Office including Excel, PowerPoint and Word Well developed presentation skills and ability to present a range of new and unfamiliar concepts to an audience Verbal and written skills to communicate with all levels of staff within the Trust Able to support junior staff within the project or wider charity team Self-motived and able to work independently with little guidance required Experience Essential A genuine desire to support underprivileged and minority groups The post will involve travelling across hospital sites, and to partner organisations or external venues for events This post will involve co-ordination of and occasional attendance at evening or weekend events.
