Key Responsibilities

  • Provide holistic assessments of patients' care needs and develop, implement, and evaluate care plans.
  • Work autonomously as a practitioner, responsible for delegating care to other team members as appropriate.
  • Collaborate with other members of the multidisciplinary team to promote independence and support timely discharge.
  • Contribute to the development and implementation of care pathways and protocols.

The South West Neighbourhood Team is a proactive and forward-thinking team that strives for excellence in patient care and modern ways of working. We work closely with various teams, including Therapy, Virtual Ward, and Rapid Response, to support multi-professional working.

Requirements

  • RGN NMC Registered
  • Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
  • Post-registration general experience
  • Experience of working in a multi-disciplinary/multi-agency health/social care environment
  • Experience and knowledge of basic administration systems, including Microsoft Office
  • Experience of successful partnership working arrangements with other services
  • Previous community experience
