About the Role
Shropshire Community Health NHS Trust is seeking a skilled and dedicated Community Staff Nurse to join our South West Neighbourhood Team. As a Community Staff Nurse, you will play a vital role in delivering high-quality patient care and supporting the delivery of excellent patient outcomes.
Key Responsibilities
- Provide holistic assessments of patients' care needs and develop, implement, and evaluate care plans.
- Work autonomously as a practitioner, responsible for delegating care to other team members as appropriate.
- Collaborate with other members of the multidisciplinary team to promote independence and support timely discharge.
- Contribute to the development and implementation of care pathways and protocols.

The South West Neighbourhood Team is a proactive and forward-thinking team that strives for excellence in patient care and modern ways of working. We work closely with various teams, including Therapy, Virtual Ward, and Rapid Response, to support multi-professional working.
Requirements
- RGN NMC Registered
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Post-registration general experience
- Experience of working in a multi-disciplinary/multi-agency health/social care environment
- Experience and knowledge of basic administration systems, including Microsoft Office
- Experience of successful partnership working arrangements with other services
- Previous community experience
