About the Job

Shropshire Community Health NHS Trust is seeking a skilled Staff Nurse to join its community nursing team. The successful candidate will provide high-quality patient care in a community setting, working collaboratively with other healthcare professionals to deliver excellent outcomes.

Main Responsibilities

  • Assess and develop care plans for patients in their own homes or chosen place of residence.
  • Deliver holistic nursing care, including administration of medications, wound care, and patient education.
  • Work collaboratively with multidisciplinary teams to promote patient independence and ensure timely discharge.
  • Contribute to the development and implementation of care pathways and protocols.

Requirements

  • RGN NMC Registered.
  • Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes.
  • Post-registration general experience, including experience of working in a multi-disciplinary/multi-agency health/social care environment.
  • Experience and knowledge of basic administration systems, including Microsoft Office.
  • Able to meet the travel requirements of the post.
