Job Title: Bank Community Staff Nurse

About the Role:

We are seeking a highly motivated and experienced Bank Community Staff Nurse to join our South West Neighbourhood Team. As a key member of our team, you will be responsible for providing high-quality patient care and support to individuals in their own homes or chosen place of residence.

Key Responsibilities:

  • Conduct holistic assessments of patients' care needs and develop individualized care plans.
  • Provide nursing care and support to patients, including wound care, medication management, and education.
  • Work collaboratively with other healthcare professionals to promote patient independence and well-being.
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improvement.
  • Contribute to the development and implementation of care pathways and protocols.

Requirements:

  • RGN NMC Registered.
  • Post-registration general experience.
