Shropshire Community Health NHS Trust has an exciting opportunity for a Band 3 Healthcare Assistant to join the South West Neighbourhood Team (ICS/IDT).

We are a proactive & forward-thinking team, who constantly strive for the delivery of excellent patient care and modern ways of working. The South West Community Nursing Team works closely with a verity of teams, including Therapy, Virtual Ward and Rapid Response supporting multi professional working.

The role of the Healthcare Assistant is imperative to the team, involving undertaking direct patient care, lone working and supporting Community Nurses and Therapists undertaking visits. The Healthcare Assistant also undertakes administrative duties within the team such as ability to manage stock ordering and stock rotation.

The post is for 22.5 hours per week.

You will need to be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development. We offer a supportive induction and supernumerary period as well as continuous professional development.

The team provides nursing care to patient within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.

You will be will be responsible for the delivery of high quality care to patients whilst working with other members of the MDT to promote independence for patients on the team caseload.

For this role you will be required to manage, prioritise and organise your day with the support and oversight of the Band 6 Caseload Holder.

**This role will require people with highly developed**: interpersonal and communication skills (both verbal and written), organisational and technical skills.
