Job Description:
As a highly skilled family medicine practitioner, you will be responsible for providing a comprehensive range of clinical duties, including consultations, home visits, and telephone queries from patients and healthcare professionals.
You will work autonomously, making professional decisions in relation to presenting problems, and undertake on-call duties as part of the surgery on-call rota, where agreed.
Key responsibilities include:
- Assessing and referring patients presenting with mental health needs in accordance with national and local guidelines
- Screening patients for disease risk factors and early signs of illness
- Developing care plans for patients in consultation with them and in line with current practice disease management protocols
- Compiling and issuing computer-generated acute and repeat prescriptions
Requirements:
To be successful in this role, you must have a strong foundation in family medicine, excellent communication skills, and a patient-centered approach.
