Job Title: Medicines Optimisation Specialist

About Us: i3 Ladywood Primary Care Network is a leading healthcare provider dedicated to delivering exceptional patient care. We are seeking an experienced Pharmacist to join our team as a Medicines Optimisation Specialist.

Salary: £60,000 - £80,000 per annum (dependent on experience)

Job Description:

We are looking for a highly skilled and motivated Pharmacist to lead the development and implementation of medicines optimisation initiatives within our Primary Care Network. The successful candidate will work closely with GPs, practice staff, and other healthcare professionals to improve patient outcomes through evidence-based medicine use.

Key Responsibilities:

  • Lead the development and implementation of medicines optimisation strategies to improve patient outcomes
  • Conduct clinical medication reviews and produce recommendations for senior clinical pharmacists, nurses, and GPs
  • Work with care home staff to improve the safety of medicines ordering and administration
  • Manage a caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence
  • Provide education and training to the primary healthcare team on therapeutics and medicines optimisation

Requirements:

  • Registered Pharmacist with the General Pharmaceutical Council
  • Experience in medicines optimisation and/or clinical pharmacy practice
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
