Job Summary
Care home pharmacist sought for a challenging role within our expanding Primary Care Network. As Meds Optimisation Specialist, you will play a key part in improving patient care through evidence-based prescribing practices. About the Role
We are seeking a highly motivated and experienced pharmacist to join our team as a Primary Care Network Pharmacist. In this pivotal role, you will work closely with general practice teams to ensure that medicines are used effectively, safely, and compassionately. Your expertise will be instrumental in identifying areas for improvement, developing and implementing strategies to address these issues, and providing educational support to primary healthcare professionals.

Responsibilities
- Conduct clinical medication reviews with patients and produce recommendations for senior clinical pharmacists, nurses, and/or GPs on prescribing and monitoring.
- Work with care home staff to improve safety of medicines ordering and administration.
- Provide education and training to primary healthcare teams on therapeutics and medicines optimisation.
- Assist practices in seeing and maintaining a practice formulary that is hosted on the practices computer system.

Requirements
- Registered with the General Pharmaceutical Council (GPhC).
- Evidence of commitment to continuing professional development.
- Previous experience in a GP practice and/or hospital environment.
