About the Role

We are seeking an experienced and passionate Social Worker to join our 14+ Leaving Care team in Northumberland. As a key member of our team, you will play a vital role in supporting young people transitioning from care into independence.

Key Responsibilities
  • Provide individualized support to young people, helping them build the skills and resilience they need for their future.
  • Work closely with the 14+ Leaving Care team to ensure a seamless transition for young people from care to independence.
  • Develop and implement personalized support plans to meet the unique needs of each young person.
  • Collaborate with other professionals, such as education and employment services, to ensure a holistic approach to support.
