We have an exciting opportunity for a Social Worker to join our Care and Support Team based at a local hub in the North East region on a permanent contract. The successful candidate will be working predominantly in our Community Allocation Hub, which is one of our pilot sites for Prevention.

We are looking for a dynamic qualified Social Worker who can think creatively, who can utilise community services where possible, to prevent or delay the need for care and support in line with the Care Act.

The post is a full-time role on a permanent contract basis.

You will complete Adult Needs Assessments, Mental Capacity Assessments and collaborate with a range of external services to access any resources required to meet the care and support needs you have identified.

It is essential that you have a clear understanding of the Care Act and the Local Authorities statutory duties. We are committed to protecting and promoting the welfare of vulnerable adults and expect all staff to share this commitment.

Key Responsibilities:

  • Social Work degree
  • Social Work registration
  • Full UK driving licence with access to a car
