Nurse in Community Care Leadership

UCLH@HOME Deputy Sister/Charge Nurse is an exciting opportunity to join our team at Whittington Health, a service provider in partnership with University College London Hospitals (UCLH). This role is a key part of integrated care, focusing on providing innovative and transformational care for local communities.
We are seeking a skilled RGN with excellent clinical and holistic assessment skills. The ideal candidate will have a leadership role, taking responsibility for case-finding, team coordination, and organization of transfers home. Our team provides direct care delivery within patients' own homes, utilizing remote monitoring technologies to ensure high-quality care.
The successful candidate will work closely with colleagues from acute specialties, emergency departments, and community services. This role offers a unique chance to be part of a truly integrated care pathway, developing and expanding services to provide the best possible care for our patients.
Main Responsibilities:
To provide care for patients transferred to the community from UCLH prior to their estimated discharge date. The post holder will have a leadership role, responsible for case-finding, team coordination, and organizing transfers home. The ideal candidate should have excellent communication skills, both written and verbal, and the ability to demonstrate good leadership skills.
Required Skills and Qualifications:
Essential qualifications include a current NMC registration, 2 years of experience as an RGN, and a portfolio of varied clinical experience. Additionally, knowledge of current issues in the NHS and experience in community, A&E, or care of the elderly settings are desirable. A nurse prescriber qualification or willingness to work towards it is also essential.
