Central London Community Healthcare NHS Trust is seeking an experienced and motivated nurse to fill the role of District Nurse Team Leader. As a key member of our Brent Community Nursing Team, you will have the opportunity to lead a team of skilled nurses and healthcare assistants in delivering high-quality patient care in a community setting.
About the Role
As a District Nurse Team Leader, you will be responsible for managing a caseload and delivering evidence-based nursing care to patients in their homes. You will also play a vital role in guiding your team, ensuring exceptional care and promoting service development through monitoring care quality and identifying opportunities for improvement.
Your Responsibilities
- Manage a caseload and deliver evidence-based nursing care to patients in their homes
- Lead a team of skilled nurses and healthcare assistants in delivering high-quality patient care
- Monitor care quality and identify opportunities for improvement
- Support the development of your team through training and mentoring
- Collaborate with other healthcare professionals to provide seamless, integrated care tailored to each patient's unique needs
Requirements
- Registered General Nurse with current UK NMC registration
- Specialist Practitioner qualification in District Nursing or working towards it
- Proven experience in community nursing with a focus on complex care and team coordination
- Strong leadership skills with the ability to motivate, delegate, and handle complex care decisions
