Bradford District Care NHS Foundation Trust invites applications for a Clinical Director of Care position in Older Peoples Mental Health Services.

About the Role

This full-time position involves working across the Dementia pathway, supporting the inpatient ward and Community Services, including the Memory Assessment Service, Care Home Liaison, and the Outreach Team.

The ideal candidate will have experience in embedding a culture of least restrictive practice and supporting the planning of discharge from admission from the two inpatient wards.

The post holder will be responsible for developing key relationships with other support services within the Trust and external partners to deliver high-quality services.
