At Bradford District Care NHS Foundation Trust, we are seeking an Older Adults Mental Health Care Coordinator to join our team. As a key member of our multidisciplinary team, you will work with older adults who have mental health conditions or dementia, providing coordinated care and support.

Key Responsibilities:
  • Support our service users and the team working with older people who have mental health diagnoses or dementia
  • Work closely with care homes, families, and other stakeholders to deliver comprehensive care
  • Collaborate with the wider multidisciplinary team to ensure seamless care coordination

The ideal candidate will have experience working with older adults with mental health needs and/or dementia. You will be passionate about driving forward quality care and committed to delivering excellent services.
