About the RoleThis position offers an exceptional opportunity for an experienced Nurse to lead in the delivery of high-quality care and support to older adults with mental health needs or dementia.
As a key member of our multidisciplinary team, you will develop and implement personalized care plans, addressing the complex needs of individuals with dementia. Your commitment to person-centered care and compassion will enable you to make a tangible difference in the lives of those you serve.
Main Duties- Provide clinical support to service users with mental health needs or dementia, working in close collaboration with the wider multidisciplinary team.
- Design and implement care plans tailored to the unique needs of individuals with dementia, prioritizing their comfort, dignity, and well-being.
- Nurture strong relationships with care homes, families, and other stakeholders, facilitating effective communication and cooperation.
- Current RMN registration on part 3 or part 13 of the NMC Register.
- Proven experience working with individuals who have dementia, ideally in a community-based setting.
- Dementia qualification (desired).
- A salary between £38,890 and £44,503 per annum, taking into account your qualifications and experience.
