Palliative Care Nurse

Position Overview
The role involves being an integral member of the Integrated Care Team, delivering direct support to children and adolescents with palliative care requirements, whether in a hospice, at home, or in a hospital setting, as dictated by the individual care needs and service provisions. The successful candidate will be expected to undertake a range of shifts, as the hospice operates around the clock, every day of the year.
PRIMARY DUTIES
Clinical Responsibilities
1. To deliver exceptional nursing care to children and young individuals with palliative care needs.
2. To advocate for a patient-centered care model within a multidisciplinary team framework.
3. To assist in the formulation of care plans and assessments to guarantee that the child or young person receives safe and effective care from hospice services.
4. To collaborate with the designated lead for mental capacity evaluations and ensure these are conducted as necessary.
5. To ensure that care is organized and executed in line with the standards set by the Care Quality Commission Inspectorate for quality and safety.
6. To be well-versed in and contribute to all clinical protocols, policies, and procedures of Haven House.
7. To convey complex and sensitive information effectively to service users, care personnel, and other team members.
8. To enhance clinical skills and knowledge while maintaining personal competency for the NMC portfolio.
9. To collaborate with the Nurse Practice Educator in providing training and further support to junior colleagues, particularly in relation to healthcare support workers.
10. To ensure that safeguarding practices are integrated into daily operations and that any concerns are raised promptly.
11. To contribute to effective medication management in accordance with Haven House policies.
12. To develop and sustain the clinical skills and knowledge necessary for delivering holistic, evidence-based nursing care.
13. To act as an advocate for the child and their family.
14. To ensure that the nutritional requirements of the child or young person are met while at Haven House.
15. To possess knowledge of commonly used medications in pediatric palliative care and to complete the relevant drug administration competencies.
16. To be knowledgeable about safeguarding policies and procedures.
17. To utilize Haven House's Electronic Patient Record System to ensure data is recorded accurately and promptly, in accordance with established policies and procedures.
18. To demonstrate a professional demeanor and adhere to the NMC Code in all aspects of work.
Collaborative Efforts
1. To proactively identify issues related to care strategies, policies, and procedures in the interest of the organization and service users.
2. To participate in the evaluation of services as required, ensuring that the perspectives of children, young people, and their families are represented.
3. To be mindful of allocated resources and ensure effective time management in one's role.
Clinical Governance
1. To engage in audits, collection of service user feedback, and research as necessary.
2. To report clinical and non-clinical incidents in accordance with established policies.
Educational Responsibilities
1. To identify personal learning needs and ensure ongoing professional development is maintained, contributing to the appraisal system.
2. To attend all mandatory training as required by the service.
3. To mentor pre-registration nursing students, providing constructive feedback to their assessors.
4. To contribute to the overall development of staff and services by updating the evidence base for clinical practice.
5. To educate and train parents and caregivers to meet the healthcare needs of their child.
6. To provide education and training to junior staff and students to address the health care needs of service users.
7. To comply with Haven House's arrangements for continuing professional development.
Professional Standards
1. To adhere to Haven House policies and the NMC Code of conduct, performance, and ethics for nurses and midwives.
2. To maintain clear and accurate documentation.
3. To uphold confidentiality at all times, in accordance with Data Protection and Caldicott guidelines.

Essential
Experience working independently and as part of a team. Ability to teach clinical skills to parents and care staff. Capability to work autonomously in various settings. Self-motivated and proactive. Proficient in English, both written and verbal. IT literate: capable of word processing and using basic spreadsheets. Flexible and adaptable to meet competing priorities. Understanding of a range of pediatric conditions, including life-limiting, life-threatening, and end-of-life care needs. Knowledge of infection control procedures. Awareness of personal health and safety responsibilities. Good understanding of clinical supervision and mentoring. Awareness of equality within the workforce and its impact on this role. Willingness to work unsocial hours as required.Desirable
Experience with a broad range of pediatric nursing needs and complex care, including technology-dependent children and childhood cancer. Experience caring for children and young people requiring end-of-life support. Technical pediatric nursing skills, such as care of enteral feeding tubes, tracheostomies, and emergency medication for seizures. IV competency. Ability to teach clinical skills to parents and care staff. Understanding of a range of pediatric conditions, including life-limiting, life-threatening, and end-of-life care needs.
