Job Summary
We are seeking a compassionate and organized Care Partner Coordinator to join our team. As a key member of our care team, you will be responsible for coordinating care plans, scheduling appointments, and ensuring seamless care delivery.
Key Responsibilities
- Coordinate care plans with healthcare professionals and family members
- Schedule appointments and manage client schedules
- Communicate effectively with clients, families, and healthcare teams
- Monitor client progress and adjust care plans as needed
Requirements
To be successful in this role, you will require:
- A strong passion for delivering high-quality care and support
- Excellent communication and organizational skills
- The ability to work independently and as part of a team
- A valid driving license and access to a vehicle
