We are seeking an experienced Care Coordinator to join our team at Leonard Cheshire UK. As a Care Coordinator, you will play a vital role in supporting our customers with disabilities to live independently.

Key Responsibilities:
  • Assist with daily life tasks, including personal care and medication
  • Escort and assist customers in and around the service, at external events and appointments
  • Support our Person Centred approach, encouraging maximum independence for our customers
  • Maintain accurate records of customer interactions
Requirements:
  • Good interpersonal skills and ability to communicate effectively
  • Flexibility to work under pressure and as part of a team
  • Ideal candidate will have personal care experience
  • Flexibility to work within rostered hours
