Clinical Rehabilitation Assistant

We are seeking a skilled Clinical Support Worker to join our Specialist Falls Team. The successful candidate will work closely with Physiotherapists and Occupational Therapists to implement falls prevention programmes in patients' homes and care facilities.
As a key member of our team, you will be responsible for undertaking independent visits to implement falls prevention plans, including exercise programs, mobility practice, and equipment provision as requested by therapists.
The role requires excellent interpersonal, communication, and active listening skills, with the ability to adopt appropriate styles and methods of communication. You will also need basic IT skills to use electronic patient records and other Trust systems.
The estimated salary for this role is £23,500 - £28,000 per annum, depending on experience.
Key Responsibilities:
- Implement falls prevention programs in patients' homes and care facilities
- Undertake independent visits to implement falls prevention plans
- Provide exercise programs, mobility practice, and equipment provision as requested by therapists
- Work closely with Physiotherapists and Occupational Therapists to develop and deliver rehabilitation programs
