About the Role

We are seeking a skilled Clinical Rehabilitation Specialist to join our team at South Tees Hospitals NHS Foundation Trust. In this role, you will work closely with Physiotherapists and Occupational Therapists to provide rehabilitation services to patients in their own homes and care facilities.

As a Clinical Rehabilitation Specialist, you will be responsible for implementing falls prevention programs and working with statutory and voluntary services to promote independence. You will also have the opportunity to develop your skills and knowledge through support and in-service training provided by the trust.

The ideal candidate will have experience working in health or social care, specifically as a therapy assistant, and possess strong communication and interpersonal skills. You will also have a working knowledge of Occupational Therapy or Physiotherapy.

Salary: £22,000 - £28,000 per annum (dependent on experience)

Key Responsibilities:

  • Implement falls prevention programs in patients' homes and care facilities
  • Work with Physiotherapists and Occupational Therapists to provide rehabilitation services
  • Develop and maintain relationships with statutory and voluntary services
  • Provide support and guidance to patients and their families
