About the Role

We are recruiting a Therapy Assistant Healthcare Professional to join our team in providing rehabilitation services to patients in the local community. As a key member of our team, you will work closely with Physiotherapists and Occupational Therapists to implement falls prevention programs in patients' homes and care facilities.

The successful candidate will have experience working in health or social care, with a strong understanding of clinical observations and procedures. A working knowledge of Occupational Therapy or Physiotherapy is also desirable.

Key Responsibilities:

  • Collaborate with healthcare professionals to develop and implement personalized rehabilitation plans for patients
  • Conduct home visits to assess patient needs and provide support with daily living activities
  • Maintain accurate records of patient interactions and progress
