About the Role

We are seeking an experienced Adult Nurse Case Manager to join our team at HFH Healthcare. As a key member of our multidisciplinary team, you will provide face-to-face support to our clients and their families, creating personalized care plans that meet their unique needs.

Key Responsibilities
  • Provide high-quality, patient-centered care to adults with complex needs, including those with spinal cord injuries, neuromuscular disabilities, and tracheostomy/ventilation requirements.
  • Work closely with a core team of carers to train and support them with clinical skills and activities.
  • Develop and implement personalized care plans that address the physical, emotional, and social needs of our clients.
  • Collaborate with healthcare professionals, including doctors, therapists, and social workers, to ensure comprehensive care.
  • Conduct regular assessments and reviews to ensure the effectiveness of care plans and make adjustments as needed.
Requirements
  • Registered Nurse with a minimum of 12 months' work experience in the UK and an active NMC PIN.
  • Tracheostomy and ventilator experience desirable.
  • ITU/HDU experience would be desirable.
  • Care of the spinal patient knowledge desirable.
  • Enteral feeding experience essential.
  • Excellent written and verbal communication skills.
  • Ability to work flexibly and respond quickly to changing priorities while meeting deadlines.
  • Ability to build strong relationships with clients, their families, and carers.
