We are seeking an experienced Adult Nurse Case Manager to join our team at HFH Healthcare. As a key member of our multidisciplinary team, you will provide face-to-face support to our clients and their families, creating personalized care plans that meet their unique needs.

With a focus on complex care, you will work closely with our core team of carers, training and supporting them with clinical skills and activities. Your expertise will be invaluable in helping us deliver advanced care and improve the lives of our clients.

Key Responsibilities
  • Provide high-quality, patient-centered care to adults with complex needs
  • Develop and implement personalized care plans in collaboration with clients, families, and carers
  • Work closely with our multidisciplinary team to ensure seamless care delivery
  • Conduct regular assessments and reviews to ensure the best possible outcomes for our clients
  • Participate in ongoing professional development to stay up-to-date with best practices and industry developments
Requirements
  • Registered Nurse with a minimum of 12 months' work experience in the UK and an active NMC PIN
  • Tracheostomy and ventilator experience desirable
  • ITU/HDU experience would be desirable
  • Care of the spinal patient knowledge desirable
  • Enteral feeding experience essential
  • Excellent written and verbal communication skills
  • Ability to work flexibly and respond quickly to changing priorities
  • Ability to build strong relationships with clients, families, and carers
