Role Overview:
As an Adult Nurse Case Manager (Tracheostomy Experienced), you will be tasked with offering direct support to clients and their families while developing personalized care plans tailored to each individual. You will collaborate closely with a dedicated team of caregivers, providing training and support in clinical skills and activities.

Key Qualifications:
- Registered Nurse with a minimum of 12 months' experience in the UK and an active NMC PIN
- Experience with tracheostomy and ventilators is preferred
- ITU/HDU experience is advantageous
- Knowledge of spinal patient care is desirable
- Essential experience in enteral feeding
- Strong written and verbal communication skills
- Ability to adapt and respond to changing priorities while meeting deadlines
- Capability to build robust relationships with clients, families, and caregivers
- Supportive management style that fosters the best in others
- Confident, passionate, and possessing a proactive attitude
- Ability to manage your own caseload of community packages, including community work and collaboration with the head office
