About the Role

We are seeking an experienced Adult Nurse Case Manager to join our team at HFH Healthcare. As a key member of our multidisciplinary team, you will be responsible for providing face-to-face support to our clients and their families, creating personalized care plans, and working closely with a core team of carers to deliver high-quality care.

Key Responsibilities
  • Provide expert care and support to clients with complex conditions, including spinal cord injuries, neuromuscular disabilities, tracheostomy/ventilation, acquired brain injuries, learning and physical disabilities, and epilepsy.
  • Develop and implement personalized care plans, taking into account the unique needs and goals of each client.
  • Work collaboratively with a core team of carers to deliver high-quality care, providing training and support as needed.
  • Communicate effectively with clients, their families, and healthcare professionals to ensure seamless care coordination.
  • Manage a caseload of community packages, working in the community and at our Wimbledon head office.
Requirements
  • Registered Nurse with a minimum of 12 months' work experience in the UK and an active NMC PIN.
  • Tracheostomy and ventilator experience desirable.
  • ITU/HDU experience would be desirable.
  • Care of the spinal patient knowledge desirable.
  • Enteral feeding experience essential.
  • Excellent written and verbal communication skills.
  • Ability to work flexibly and respond quickly to changing priorities while meeting deadlines.
  • Ability to build strong relationships with clients, their families, and carers.
