About the Role:

We are seeking a highly skilled Nurse Case Manager to join our team at HFH Healthcare. As an Adult Nurse Case Manager, you will be responsible for providing face-to-face support to our clients and their families, creating personalized care plans, and working closely with a core team of carers to train and support them with clinical skills and activities.

Key Responsibilities:

  • Providing specialist nurse-led services to adults and children in their own homes
  • Working in partnership with the NHS continuing healthcare teams
  • Creating personalized care plans for each client
  • Training and supporting carers with clinical skills and activities

Requirements:

  • Registered Nurse with a minimum of 12 months' work experience in the UK and an active NMC PIN
  • Tracheostomy and ventilator experience desirable
  • ITU/HDU experience would be desirable
  • Excellent written and verbal communication skills
  • Ability to work flexibly and respond quickly to changing priorities
