As a Adult Nurse Case Manager, you will play a vital role in our team, working closely with clients, their families, and carers to create personalized care plans and provide face-to-face support.

Key Responsibilities

  • Develop and implement comprehensive care plans for clients with complex needs
  • Provide expert clinical support and training to carers
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery
  • Manage a caseload of community packages, including working in the community and at our Wimbledon head office

Requirements

  • Registered Nurse with a minimum of 12 months' work experience in the UK and an active NMC PIN
  • Tracheostomy and ventilator experience desirable
  • ITU/HDU experience would be desirable
  • Enteral feeding experience essential
  • Excellent written and verbal communication skills
  • Ability to work flexibly and respond quickly to changing priorities
  • Ability to build strong relationships with clients, their families, and carers
