About the Role

We are seeking a skilled and compassionate Adult Nurse Case Manager to join our team at HFH Healthcare. As a key member of our multidisciplinary team, you will provide face-to-face support to our clients and their families, creating personalized care plans that cater to their unique needs.

Key Responsibilities
  • Deliver high-quality, patient-centered care to adults with complex needs, including those with spinal cord injuries, neuromuscular disabilities, and tracheostomy/ventilation requirements.
  • Work closely with a core team of carers, training and supporting them with clinical skills and activities to ensure seamless care delivery.
  • Develop and implement comprehensive care plans that address the physical, emotional, and social needs of our clients.
  • Collaborate with healthcare professionals, including GPs, consultants, and other specialists, to ensure coordinated care and optimal outcomes.
  • Conduct regular assessments and reviews to monitor client progress, identify areas for improvement, and adjust care plans accordingly.
Requirements
  • Registered Nurse with a minimum of 12 months' work experience in the UK and an active NMC PIN.
  • Desirable: Tracheostomy and ventilator experience, ITU/HDU experience, care of the spinal patient knowledge, and enteral feeding experience.
  • Excellent written and verbal communication skills, with the ability to build strong relationships with clients, their families, and carers.
  • Ability to work flexibly and respond quickly to changing priorities while meeting deadlines.
