About the Role

The Royal Devon and Exeter NHS Foundation Trust is seeking a highly motivated and enthusiastic Rehabilitation Support Worker to join our therapy team at Sidmouth Community Hospital. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the hospital's mission to provide exceptional healthcare services.

Key Responsibilities
  • Carry out rehabilitation groups and individual programmes of intervention devised for patients by qualified therapy staff, with the aim of increasing patient independence, reducing deconditioning, and facilitating discharge from hospital.
  • Work collaboratively with a supportive, integrated multidisciplinary team, including Physiotherapists, Occupational Therapists, Support Workers, and Nursing staff, to deliver comprehensive patient care.
  • Develop and maintain a strong understanding of community rehabilitation, multi-disciplinary working, falls prevention, and management of patients with a wide variety of short and long-term conditions.
  • Contribute to the development and implementation of treatment plans within your competencies or as delegated by registered staff.
  • Participate in clinical supervision and peer support to ensure high-quality patient care and professional development.

  • A strong interest in community rehabilitation, multi-disciplinary working, falls prevention, and management of patients with a wide variety of short and long-term conditions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • A flexible and adaptable approach, with the ability to work in a fast-paced environment and respond to changing patient needs.
  • Basic computer skills and ability to use electronic patient records and other healthcare systems.
